Please only complete these surveys IF you have been instructed to do so either in your appointment letter or by your surgical team.

Patients undergoing joint replacement surgery throughout the province are invited to complete a survey regarding their experience with joint replacement at their New Consult appointment, 6 week and 12 week post-operative check-ups and their one year follow-up appointment. Please select and complete the survey that applies to you:

• This will be my first visit for this problem (New Consult appointment)
  • This survey is not to be completed after your assessment appointment (the first appointment you have with a doctor after you have been referred to our clinic).
  • This is to be completed prior to your first appointment with a surgeon once you have been deemed a candidate for surgery by one of our assessment physicians.
• I had surgery about 6 weeks ago
• I had surgery about 3 months ago
